Two employees were installing a boom on a bucket truck. The boom was on a chain attached to a forklift; one employee was operating the forklift while the other moved the boom onto the truck. The chain released and the boom fell to the ground, smashing the employee's right hand under it. The employee suffered fractures to the middle and ring fingers and a partial amputation to the index finger.
